You can schedule an appointment online for a iCAT scan only at the West Valley Professional Center location using our automated scheduling system. Please be sure to have your Doctors name and your credit card information before you start the process. Credit card information is used to hold the appointment date and time and is not processed until the day of the appointment. Please note there is a 50% fee for missed or failed appointments. If you need to reschedule, please call or reschedule online 24 hours prior to your appointment. If you are a Doctor and need referral tags, you can download them from this page. Please call our office if you'd like us to send you a supply of referral tags.
